Program Accessibility

The regulation implementing section 504 requires that an agency/facility "shall adopt and implement procedures to ensure that interested persons, including persons with impaired vision or hearing, can obtain information as to the existence and location of services, activities, and facilities that are accessible to and usable by disabled persons." (45 C.F.R. 84.22(f)).

Access Notice

This facility and all of its programs and activities are accessible to and useable by disabled persons, including persons with impaired hearing and vision.

Access features include:

  • Convenient off-street parking designated specifically for disabled persons.
  • ramp between parking areas and building walkway.
  • Level access into the building. This is not a multi-level building.
  • Fully accessible offices, treatment rooms, bathroom facility, gym equipment.
  • Fully accessible pool lift for those patients who can not accend or decend stairs.
  • Assistive and communication aids provided to persons with impaird hearing, visual, speech, and/or manual skills.

Some of these aids include:

  • Assistance by our office personell in reading and filling out patient intact forms if/as needed. 
  • Maintain privacy and conficentiality to you and family members at all times during your treatments.
  • a twenty-four hour (24) telecommunication interpretator through AT&T company as needed.
  • Flash Cards, Alphabet boards and other communication boards.
  • Assistive devices for persons with impaired manual skills ie.. wheelchair usage, handicaped elevated commode seat, electric lift exam table, among others.
